What Tests Should Women Have Before Planning a Pregnancy?

Planning a pregnancy isn't just about choosing the right month to start trying. It’s also about knowing what’s happening inside your body before those two lines appear on a pregnancy test.

A little preparation can go a long way.

Some women need only a few basic checks. Others may benefit from more detailed testing depending on their age, menstrual cycle, medical history, previous pregnancies, or fertility concerns. This is where a preconception consultation with a gynaecologist can make things much clearer.

So, what tests should women consider before planning a pregnancy?

1. Complete Blood Count

A simple blood test, but a useful one.

A Complete Blood Count (CBC) can help identify anaemia and provide information about your general blood health. Iron deficiency is fairly common among women, particularly when menstrual bleeding is heavy, and correcting it before pregnancy can be helpful.

Your doctor may recommend iron, folic acid, or other supplements based on your results.

2. Blood Group and Rh Factor

Knowing your blood group and Rh status before pregnancy is useful.

If a woman is Rh-negative and her partner is Rh-positive, doctors may recommend additional monitoring during pregnancy because of the possibility of Rh incompatibility. It sounds complicated. It isn't, once your doctor explains what it means for you.

3. Blood Sugar Testing

Pregnancy changes the way the body handles glucose.

A fasting blood glucose test and, where appropriate, an HbA1c test can help identify previously undiagnosed diabetes or blood sugar concerns before conception. This becomes particularly relevant if you have a family history of diabetes, are overweight, or have experienced irregular periods or previous pregnancy complications.

Getting blood sugar under appropriate control before conception is often much easier than trying to sort it out after pregnancy has already begun.

4. Thyroid Function Tests

The thyroid may be small, but it plays a surprisingly big role in reproductive health.

A doctor may recommend a TSH test, with additional thyroid tests when indicated. Thyroid disorders can affect menstrual cycles, ovulation and pregnancy, so identifying a problem early gives you time to manage it properly.

5. Tests for Infections

Depending on your medical history and vaccination status, your gynaecologist may recommend screening for infections such as hepatitis B, hepatitis C, HIV and syphilis.

Tests for immunity to rubella and varicella may also be considered. If you're not immune to certain infections, vaccination may need to happen before pregnancy rather than during it.

Timing matters.

6. Pelvic Examination and Ultrasound

Not every woman needs an ultrasound before trying to conceive. But when there are irregular periods, pelvic pain, previous fertility issues, fibroids, PCOS or suspected ovarian problems, an ultrasound can provide valuable information about the uterus and ovaries.

Your doctor may suggest other fertility-related tests if your history indicates a need.

7. Genetic or Other Specialised Tests

Sometimes additional testing makes sense because of family history, recurrent pregnancy loss, previous affected pregnancies, or certain genetic conditions. These aren't routine tests for everyone, and that's precisely why they should be guided by a doctor rather than ordered randomly.

Give Your Pregnancy a Better Starting Point

Think of preconception testing as a health check before a major journey. You don't wait for the car to break down halfway through the trip to check the engine.

At Shivam IVF, women can discuss their individual reproductive health and pregnancy plans with an experienced fertility team. Dr Bhavana Mittal can help determine which tests are actually relevant to your situation rather than sending you home with a long, confusing checklist.

For women looking for a Best Gynaecology Centre in Delhi, the right approach isn't simply about doing more tests. It's about doing the right tests, at the right time, for the right reasons.

And sometimes, that's the smartest preparation of all.
